You can get a soft copy of your policy document by logging in to MyAviva :

* Once you login, click on the “My e-statements” tab.

* Choose “Policy Document” in the field “e-statements for”.

To get the duplicate hard copy of your policy document, please submit following documents at any Aviva branch.

* Indemnity on Rs.100 stamp paper (Format available at any Aviva branch)

* Photo ID Proof

* Recent coloured photograph

* Endorsement Form

* Bank statement showing the remittance proof ( Applicable for NEFT/ECS/Cheque cases ), for DD/Cash cases copy Original ID proof ( Given at the time of policy ) to be submitted & verified by the branch executive

* Cheque leaf copy

You will have to pay an issuance fee of Rs.295/-(including GST) either through NEFT or by cheque

However, if you still wish to cancel the policy during the Free-look period, please submit the following documents at any Aviva branch or courier it to Aviva’s Branch/ Head Office:

* Payout Form duly filled and signed by the Policyholder

* Original Policy Document

* Photo Identity Proof (in case of courier, the copy needs to be attested by Govt Gazetted Officer)

* Recent Colored Photograph

* Original Cancelled Cheque with pre printed name and account number or Bank Statement/Passbook having pre-printed name & account number (in case of courier, the bank statement copy/ passbook copy needs to be attested by Govt Gazetted Officer)

For renewing your policy, you need to pay the outsanding premium due.

You might also need to submit Declaration of Good Health (DGH) Form if your policy satisfies any of the following criteria.

* If your policy has been in the lapsed state for more than 6 months (not applicable to pension products)

*  If any of the Health Riders is attached to your Policy

* If the product is “Aviva Health Secure” or “Aviva Health Plus”
